Shamim Islam M.D

Dr. Islam is a pediatrics specialist who has over 30 years of experience in the medical field. As a pediatric physician, she is committed to providing the highest quality of medical care to her young patients and their families. Above all, she believes that building strong relationships with her patients is essential to providing the best possible care.
OFFICE POLICIES
Prescription policy:
- ADHD/ADD patients require to come in for a revaluation every three months for refill requests.
- Prescription requests/refills take up to 24-48 hrs
Financial policy:
- Co-payments: co-payments are expected to be paid at the time of service.
- Newborns: We realize that it may take some time for newborn babies to be added to an insurance policy, so we offer a 1 month grace period.
- Account balances: If you have a deductible, coinsurance , or non-covered service, you will receive a bill in the mail. If the bill is not paid over a 60 day period, you will be put on a bad debt list and the bill may be sent to collections.

- No-show policy: We strive to have available appointments to meet the needs of all our patients. Missed appointments negatively impact our ability to care for your child and others. A 15$ no-show fee will be charged to your account with any missed appointment.
Three no-show appointments may result in dismissal from the practice.
- Cancellation policy: Please allow us to be better prepared by giving us a 24 hour notice for any cancellations or rescheduling requests.
- Medical records: Should you require a copy of your child's medical record, you may obtain it by calling our office. The fee breakdown is as follows (fees subject to the Medical Record Access Act by State of Michigan):$28.92 initial fee$1.45 per page for the first 20 pages$0.72 per page for the next 21-50 pages$0.29 per page for anything over 51 pages
